Routine Office Visits

More than one business day's notice: No Charge

Less than one business day's notice: $50.00

Provider may request that you reschedule if you arrive more than 10 minutes late for your appointment.

In-office Surgery

More than 24 hours notice: No Charge

Less than 24 hours notice: $100

Surgeon may cancel your procedure if you arrive more than 30 minutes late.

Hospital Admission (Surgery/Induction/Cesarean)

 $100/hour until your arrival.

 Provider may cancel your procedure if you arrive more than 60 minutes late.

Repeat Occurrences

Three (3) no-call/no-show appointments of any type will result in dismissal from the practice and no further visits may be scheduled.

Three (3) rescheduled appointments with less than 24-hours notice may result in dismissal from the practice and no further visits may be scheduled.

This policy applies to both new and established patients.
